The
Transition Project continues to target young people aged 13 to 25
years, with a learning disability who attend schools or colleges
across
North
Staffordshire
.
This project offers a means by which young people
with a learning disability can express their own views and
expectations at a crucial time in their development bridging the
divide between children and adult services.
Referrals
to the advocacy service can be made directly by the young person, by
their parent/carer, teacher, social worker or any other individual
involved in their life.
The
work of the transition advocate is varied and concentrates on the
issues that the young person themselves feel are important.
This on occasion can greatly differ from the issues that
professionals involved in the young person’s care may want to
address.
